Details of the Plan
Our affordable discount plan was created to give greater access to quality, affordable dental care to those without dental insurance. When you enroll in the plan, preventative and diagnostic care is included at no extra cost. In addition, restorative and cosmetic dentistry is available at a greatly discounted price.
The following services are included in the plan at a 100% discount:
Exams
- Comprehensive at initial visit
- Periodic (semi-annual)
- Limited, problem-focused once a year
X-rays
- Complete series or Panorex once every 3 years
- Bitewings once a year
- Intraoral periapicals
Cleaning
- 2 per year for adults and children
Fluoride
- 2 per year for children under the age of 18
Adult fluoride and periodontal maintenance are available at a 50% discount as part of the plan. All other services not mentioned above are offered at a 20% discount. The patient portion for each service is due at the time of service. If you have questions about any of our services, don’t hesitate to call!
The discounts of this plan can’t be combined with other discounts, promotion, or insurance coverage. This plan is not dental insurance and it is only good at Klement Family Dental.
Enrollment Fees
The costs for our affordable dental plan are as follows:
Single – $246/year
Dual – $476/year
Family – $736/year
The family plan includes up to 4 immediate family members living in the same household, including children up to the age of 18, or 22 for full-time students living in the same household. Additional family members may be added for $99/year.
The yearly fee is non-refundable and non-transferable, and the 12-month period begins on the date of purchase.
